Children's Music Camp Registration Form

I hereby give permission for my child(ren) to participate in Children’s Music Camp (August 19-22, 2024) and release Westminster Presbyterian Church and the music ministry leadership in West Chester, PA from any and all liability to me or my child(ren) as a result of their participation. Also, I understand that Westminster Presbyterian Church or the music ministry leadership does not assume any responsibility for loss or, or damage to, personal property of participant(s).

I grant permission to the music and church leadership & staff to take photos and video recordings for promotional and ministry related purposes only (e.g., website, Facebook, flyers, etc.) with the knowledge that my child(ren)’s name(s) will not be used in connection with their photograph/video.

I give consent for Westminster Presbyterian Church to administer first aid procedures in the case of minor incidents.

In the case of an emergency involving my child(ren), and no contact indicated above can be reached, I give permission to the church personnel to arrange for my child(ren) to be taken to the emergency room. I also authorize the hospital medical personnel to administer any medical care.
